Our end-of-life and palliative care focuses on improving the quality of life for our service users who have a life-limiting illness. Our Staff are trained in End-of-life care and will offer person-cantered care and support in the clients homes.

The support will concentrate on helping you manage symptoms and providing comfort and assistance. This includes help with emotional and mental health, spiritual and social needs, as well as practical assistance with daily activities and care support.

Our end-of-life and palliative care service is based on person-centered care. This means that:

  • Our trained, friendly and empathetic staff will place you at the center of planning and decision-making regarding your end-of-life and palliative care.
  • In delivering support, we will integrate the perspective of personalized care and support planning by continuously seeking and actively listening to your views and wishes.
